Patient Instructions:
  • Present a VALID ONTARIO HEALTH CARD at every visit.
  • Fully complete intake questionnaires and submit online or return prior to first visit.
  • Allow 1.5-2 hours for the first appointment.
  • Arrive 15 minutes prior to appointment for registration.
  • Bring any prior assessment reports, school report cards, Individual Education Plan, or other relevant documentation.
  • If there are any child custody arrangements, relevant documentation (ie. court orders) is required prior to accessing services.
  • Complete the Parental Agreement Form and return at first visit.
  • Patients on medication require close monitoring and follow-up and therefore are responsible for keeping their scheduled appointments within their prescription allowance time.
  • Provide the clinic notice of a need for a prescription renewal 2 weeks prior to running out of medication.
