• If you need to cancel or reschedule your appointment, please provide 24 hours’ notice.
• When you arrive, please check in with reception and bring your health card.
• Inform reception of any changes to your name, address, or phone number.
• Please arrive on time for your appointment. If you arrive 10 minutes or more late, your appointment may need to be rescheduled.
• If your lab results are normal, we will not contact you, but you can discuss them with the Nurse Practitioner at your next visit.
• For prescription renewals, some may require an appointment with the Nurse Practitioner. In certain cases, we may ask you to contact your pharmacy and have them fax a renewal request to the Clinic. Please allow 2-7 business days for processing once we receive the request from your pharmacy.
• If you need letters or forms completed for school, work, or insurance purposes, an appointment is required. Please note that completion may take up to 30 days or longer.
