• Allow 20 minutes or more to complete pre-visit check-in information online.
• Bring insurance cards and ID if new patient or significant changes.
• Bring significant medical information changes not updated in the portal.
• Consider writing a brief summary (up to 1 page) of condition and health goals.
• Bring payment if not paid during web check-in.
• For in-person visits, check Office Status sign beside the door and wait quietly.
• For telehealth visits, click the link in the text/email about 20 minutes before the scheduled visit.
• Use the patient portal for appointments, messages, bills, and records.
• Plan ahead for prescription refills, as they are provided until the next planned follow-up.
• Contact clinic by phone or portal if unexpectedly out of medication.
• Do not expect pain narcotic or benzodiazepine prescriptions from this clinic; see Primary Care or specialist for these.

The clinic is on the second floor of the Larrabee Square building in the Fairhaven neighborhood. Ground level handicap access is available from the alley/parking lot behind the building.

Established patients can make appointments by logging into the clinic’s secure patient portal or by calling the office. New patients need to establish with the clinic first by following instructions on the Help for Patients page.
