• Please make sure to get prescriptions for your chronic medication during your visit to last until your next scheduled visit.
• Please make sure to get refills before you run out.
• If your pharmacy sends us a faxed prescription request it may not be done for up to 72 hours so plan accordingly.
• Please cancel appointments 24 hours in advance or a service charge may apply.
• Please call if you will be more than 15 minutes late.
• If you expect us to accept your insurance payment for the office visit, please bring your insurance card.
• Confirm that we do participate in your insurance plan before your appointment, as you are financially responsible for the fee if we are not on your insurance panel.
• Co-payments are due at the time of the visit for insurances that have a fixed co-payment.
• For Medicare, you will be billed for your deductible and co-payment when we receive the "Explanation of Benefits" from Medicare.
• Patients on chronic narcotic prescriptions for non-cancer pain will be asked to sign a "Pain Contract" which will include random urine testing.

Exit from I-5 on Old Fairhaven Parkway (exit 250) toward Fairhaven. Turn Right on 12th Street (third light). Turn Left on Larrabee Ave. (first street). The clinic is the 4th building on the left.

By appointment: Monday - Friday 8:00 am to 5:00 pm. Walk-in Clinic: Saturday and Sun 9:30am to 4:00 pm.
