• Complete Care Medical Group forms beforehand to reduce paperwork time.
• For DOT physicals, bring your driver’s license, documentation from your Primary Care Provider regarding significant medical issues, and glasses if needed.
• For DOT physicals, it is useful to bring progress notes from recent office visits with your Primary Care Provider.
• Self-pay patients must pay in full at the time of service.
• Insured patients with verified coverage must pay applicable copayment at the time of service.
• Patients are responsible for charges not covered by insurance, including deductibles or coinsurance.
• Contact your insurance company for questions regarding your specific plan or benefits.
