  • For children who use our playroom, parents are asked to help children wash their hands before entering.
  • If you or anyone in your family is experiencing flu-like symptoms (cough, sore throat, fever or just not feeling well), please call to reschedule your appointment.
  • If you decide to stay home due to illness, please contact your therapist about alternatives to in-person therapy.
  • Please use ONLY black bold ink on all paperwork.
  • Please bring all completed forms to your first appointment.
Physical Directions:

Bremerton Office: From Silverdale heading South on SR-303, turn Right on Fairgrounds Rd., then Left onto Old Military Rd. Turn into the first driveway on the left, the Meadowdale Professional Buildings parking lot. From Bremerton heading North on Warren Ave., turn Left on Fairgrounds Rd., then Left onto Old Military Rd. Turn into the first driveway on the left, the Meadowdale Professional Building parking lot. CCFT is located in Suite 103 near the center of the building. Port Orchard Office: From Highway 16, take the Tremont exit (also known as Lund) and head toward Port Orchard. Follow Tremont approximately .7 miles. Turn right at the driveway just before the fire station. Turn right into the parking lot. Enter the entrance at the center of the building. Head left down the hallway to CCFT. CCFT is located downstairs.
