- No other appointment will be booked until full payment is received for a missed/cancelled appointment.
- If you booked online, use the same method to cancel or fill out the cancellation form.
- Existing patients can request another doctor appointment through their private secured patient portal (AVA).
- If you do not have your portal set up, fill out the unsecured form and expect a response in 72 hours.
