Patient Instructions:
- Report work accidents and injuries to employer as soon as possible.
- Tell doctor about work injury for WCB reports.
- Complete the Worker’s Report of Injury form and submit to WCB.
- Attend regular appointments as recommended by Physical Therapist to avoid discontinuation of WCB claim.
- Report car accidents and any injuries to insurance company within 10 days or as soon as possible.
- See a primary health practitioner (Physiotherapist or Family Physician) for initial assessment and treatment after a car accident.
- Complete a Notice of Loss & Proof of Claim form (AB-1) and return to insurance company within 10 days (for Diagnostic & Treatment Protocol) or 30 days (for Section B Protocols) from the date of accident.
- Exhaust extended health benefits coverage prior to accessing coverage under Section B for car accident injuries.
