Patient Instructions:
  • All participants are required to take an active role in their recovery journey.
  • Attend and engage in all sessions.
  • Respect the processes and healing experiences of fellow program members.
  • Missing two sessions will result in a warning, and three missed sessions will lead to early termination from the program.
  • Participants over the age of ten must complete daily chores.
  • Everyone is expected to contribute to keeping common areas tidy.
  • Family rooms should be maintained in a clean condition.
  • Outside of sessions, parents are accountable for their children's behavior.
  • Bring prescription medications in properly labeled blister packs (to be stored in a locked medication cabinet).
  • Bring indoor and outdoor footwear.
  • Bring toiletries, including shampoo, soap, toothbrush, toothpaste, brush, feminine care products, etc.
  • Bring essential baby items such as diapers and bottles.
  • Bring appropriate seasonal clothing: Summer: Lightweight clothes, a sweater, swimsuit, mosquito repellent, sunscreen, and hats. Winter: Boots, jackets, mittens, and hats.
  • Do not bring any illicit drugs, alcohol, weapons, gang-related clothing or colors, apparel with obscenities or drug and alcohol logos or slogans, video games, valuable items, or music or videos containing explicit lyrics.

Otter Home Referral Information

Referral Process: Individuals seeking attachment programming are required to obtain a referral from an external source. The Coordinator will conduct an assessment via phone call, TEAMS/Zoom involving the potential participant and their referral source. This process will take at least 30 minutes and should take place in a private setting. Funding must be secured before enrolling in the program.
How to Refer: Referrals can be initiated by any professional who is supporting the potential participant and their family members. To complete the referral process online, contact the clinic through the contact form on their website. Once the request is received, access to a digital Intake Package will be provided. The Intake Package can then be filled out and submitted electronically or faxed to 204-703-3000.
