• Bring insurance identification card.
• Payment for insurance co-pay is required at the first visit (checks and credit cards accepted).
• Bring referral if required.
• Bring previous x-rays, MRIs, and/or reports.
• Bring a complete list of all medications.
• Complete pre-registration through Phreesia prior to visiting.
• Provide 24 hours' notice for appointment cancellations.
• Contact pharmacy two days prior to running out of medication for refills.
• For non-emergent medical matters during office hours, call and leave a message for your physician.
• For urgent problems after hours, contact the on-call physician through the answering service.
• If insurance requires a referral, obtain it prior to the appointment.
• Bring any previous X-Rays, MRIs, CTs, Operative Reports, and notes from previously seen physicians to the appointment.
• Allow 7-10 working days for tests or procedures to be authorized and scheduled.
• Hand carry films (e.g., MRI) to the appointment.
• Call one week prior to needing X-Rays.
• See Medical Records page for instructions on obtaining medical records.
• For billing questions, call 425.339.2433, Option 4.
• Bring current health insurance card to appointment.
• Payment is expected at the time of service unless prior arrangements have been made.
• Self-Pay visits require a deposit of $300.00 for new patients and $200.00 for returning patients.
