  • If your infant was born off-post, please call the clinic to schedule the first appointment and coordinate to send a copy of baby's discharge note.
  • If you or your child is ill, please call to re-schedule the appointment.
  • For 2- to 17-year-old well physicals and sports physicals, if your child has been seen in the last year, paperwork can be completed without an appointment via the MHS GENESIS Patient Portal. Please make sure to complete the parent portion first.
  • For Immunization Only visits, please bring your child’s immunization record.
