Patient Instructions:

Arrive 10-15 minutes early for your first visit to complete a form, or download and print the New Patient Health Questionnaire beforehand.
Bring your up-to-date medicare card to each visit.
Bring all medications (including over-the-counter medications) to every appointment.
Notify the office by 12 noon the day prior to your appointment if you are unable to attend to avoid a $20 missed appointment fee.
Inform the receptionist when booking if you are traveling a long distance or believe your concerns will require more time, so an extended appointment can be booked.
Inform the clinic when booking an appointment for counseling, stress management, or mental health concerns to ensure appropriate time is booked.
Call the clinic first for after-hours access instead of going to a walk-in clinic to ensure care is part of your medical chart.
If admitted to hospital, direct questions related to in-hospital care to the on-call physician within the team.

Located on the 2nd floor, above the Lawtons pharmacy in the Regent Mall. Access is via stairs or elevator through the Lawtons store.
