Patient Instructions:

Clean your hands often using alcohol-based hand rub when entering/exiting patient rooms, coming in contact with someone, coughing, or putting on/taking off PPE.
Self-screen for respiratory symptoms by reading signage upon hospital entry.
Do not visit if unwell, had COVID-19 in the last 10 days, or close contact with someone who had COVID-19 in the last 10 days.
Follow signage for masking; masking is optional and strongly encouraged, mandatory on outbreak units.
Bring your Ontario Health Card and pertinent insurance information for registration.
Wear the identification bracelet at all times.
Bring your own toiletries (toothbrush, toothpaste, hand soap, shampoo, tissues, deodorant, razor and cream, comb and brush).
Bring your own pajamas, a robe, and slippers.
Bring any aids you use (canes, walkers, wheelchairs, hearing aids, dentures, eyeglasses) with your name labeled on them.
Bring all current medications in their original containers.
Patients or visitors choosing to bring personal items do so at their own risk, as the hospital does not take responsibility for lost, stolen or damaged belongings or assistive devices.
If requesting a private or semi-private room, present a complete insurance policy card and complete a Responsibility for Payment form.
Verify your level of coverage with private insurers.
If your condition permits, refer to the daily menu for meal choices and check with your nurse about specific meal times.
Inform staff if you follow a special diet or wish to talk to a dietitian.
