Patient and Visitor Experience for Pre-Op Clinic

Best Fit Patients:Patients requiring pre-operative assessment and testing for scheduled surgeries to ensure readiness and prevent delays.
Patient Instructions:
  • Bring forms (Pre-Registration Form, Patient Questionnaire from Department of Anesthesia) filled in prior to appointment.
  • Bring a list of all medications you are taking.
  • Bring your OHIP Health Card and all other insurance information.
  • If you have a pacemaker, bring your pacemaker card.
  • Do not bring any valuables with you as the hospital cannot be responsible for lost or stolen personal items.
  • Bring your Health card to all visits.
Physical Directions:

The Pre-Op Clinic is located on Level 0, in the Medical Outpatient Department at St. Joseph’s Healthcare Hamilton – West 5th Campus. It is recommended to park in the designated Patient Parking Lots 1 and 2 and proceed to the Medical Outpatient Entrance - level 0. If Parking Lots 1 and 2 are full, park in Lots 3 or 4 and proceed to the West 5th Street entrance and take the stairs or elevator to the Medical Outpatient Department - level 0.

Referral Process: Most elective surgical patients will be required to attend the Pre-Op Clinic. Referrals for Robotic Thoracic Surgery should be faxed to the Thoracic Surgery Common Referral Office. Bariatric surgery referrals are accepted by a referring physician through the Bariatric Registry.
How to Refer: Fax referral form for Robotic Thoracic Surgery. For Bariatric Surgery, physicians refer patients to the centralized referral portal.
