Best Fit Patients:Individuals and families within the Northern St’át’imc Territory seeking culturally sensitive health care services, including addiction and mental health support.
Patient Instructions:
  • A request for services must first go through a client intake process to match each client with an appropriate counsellor.
  • Persons requesting counselling services from SOHS must complete the referral form.
  • The Mental Outreach Health Clinician is available to help complete the form (Phone 250-256-7523), or you may complete it and email it or drop off to the address.
  • Once the Mental Outreach Health Clinician has received a signed Incoming Referral Form from a client, they will bring the request for services to the SOHS counselling team to match the client with the most appropriate counsellor, and there may be a waitlist.
  • Within 3 business days of receiving a signed Incoming Referral Form, the Mental Outreach Health Clinician will contact the client to discuss the options, which may include being placed on a waitlist and/or being referred to another counsellor while they wait.

How to Book:

Patients must complete a referral form. The Mental Outreach Health Clinician can assist with this process via phone or email, or the form can be dropped off.
