Patient Instructions:
- Patients with out of province health cards will be required to make a payment to Middlefield Medical Centre and then will be provided with a reimbursement form and a receipt of payment so that they can be reimbursed from their province of health coverage.
- Travelers with private insurance or uninsured patients will be charged a flat fee of $75 at registration along with any additional fees as required for procedures. A receipt of payment will be provided.
- Completion of a travelers insurance form is a separate charge above and beyond the above visit fee.
- For WSIB visits, please bring completed WSIB form.
- All patients are expected to come back to Middlefield Medical Centre for follow-up on test results as appropriate.
- Test results of any kind cannot be given over the phone. The patient must review the results with the doctor in person.
