• 24 hours required to cancel or re-schedule appointments
• Missed appointments will be billed
• Clearing of fees owing must be done by the patient before they can be seen by any physician
• Strollers are not permitted in the office
• Let the receptionist know the reason for your visit
• Inform receptionist if requesting a complete physical or pap smear for longer booking
• If you cannot make your appointment, change or cancel it 24 hours in advance to avoid a no show fee of $50-$80
• Test results are not given over the phone; clinic will only call if results are abnormal
• There will be a charge for photocopying and admin time if you would like a copy of your results
• Inform the receptionist of any forms needing to be filled out prior to seeing the doctor
• Ensure your file is kept up to date with current phone number, cell phone number, address, and emergency contact information
• Inform the receptionist of any changes to your personal information upon arrival
