1. 1Masks are optional in common areas as of March 17, 2025, except in areas where many patients may be at higher risk of illness.
2. 2For online booking with your primary care provider, you may book a 15-minute in-clinic appointment for new concerns or follow-up visits. Physicals, well-baby visits, or procedures are not available for online booking.
3. 3For urgent issues in the next 2-3 days, appointments longer than 15 minutes, if you do not have an OHIP number, or need an appointment with an allied health provider, please call the clinic.
4. 4For prescription renewals, ask your pharmacy to fax a medication renewal request. Urgent prescription requests cannot be accommodated over the phone.
5. 5Submit photos or documents requested by your healthcare provider using the online form. Accepted formats are JPEG, PNG, PDF, text, and standard media files, max 10MB per file.
6. 6Self-report vaccinations using the online form.
7. 7Update OHIP card details or contact information using the online form.

1. 1Bathurst site is located at the southwest corner of Bathurst Street and College Street.
2. 2Garrison Creek is located on the north side of St. Clair Avenue West, east of Oakwood Avenue at Robina Avenue and St. Clair Avenue WeSt.

Patients can book online for 15-minute in-clinic appointments for new concerns or follow-up visits. For other appointment types, patients should call the clinic.
