Patient Instructions:
- Bring valid health card.
- Wear appropriate clothing (shorts for lower extremity injury, sleeveless top for upper extremity injury).
- Bring any reports of investigations already done.
- Bring a copy of the referral from family physician (if applicable).
- Call the clinic to book with a doctor.
- A referral is required to be seen by a Sport Medicine Physician.
- 24 hours notice is required to cancel all appointments, otherwise a $50.00 no show fee will be charged.
- Missed appointments for Concussions or Orthopedics will be charged $100.00.
- Physiotherapy assessment is required on first visit, for new injury, or to re-assess existing injury post 60 days.
Physical Directions:
Located in the Ice House arena building on the campus of Carleton University. If entering off Bronson Ave, stay left on University Drive. Turn at the first left into parking lot number 5, or park at meters on University Dr. Go left after the main entrance at the Sport Medicine Clinic sign and follow the ramp to the clinic.
How to Book:
Patients must call the clinic to book with a doctor. Online booking is available for OSU players at the OSU Clubhouse.
