• Arrive 15 minutes before your scheduled appointment for vital signs and information gathering.
• Provide 48-hour notice for missed appointments to avoid charges.
• If arriving more than 5 minutes late, the clinic may not be able to see you as planned.
• Ensure your health card is valid for services covered by OHIP.
• Be aware that some services are not covered by OHIP and will be billed directly.
• Allow 30 business days for forms and certificates to be completed.
• Check the number of remaining renewals for prescriptions before each visit.
• Make an appointment before your drug prescription expires.
• Do not go to other walk-in clinics to avoid penalizing the team.
• Make an appointment to discuss any concerns regarding exam results.
• Make an appointment to be referred to a health professional or specialist.
• Advise the clinic if an appointment with another professional or specialist has not been confirmed one month after your request.
• You are responsible for paying fees imposed by specialists for missed appointments.
• Do not attempt to send information to the clinic via email as incoming messages are not received.
• Use a personal, password-protected email address for communications.
• Do not use a work email address for communications.
• Do not use email in an emergency; call 9-1-1 or go to the nearest hospital emergency room.
