- Allow a minimum of 10 business days for forms to be completed; some may take 6-8 weeks.
- Ensure all patient information is completed on forms before your physician or nurse practitioner receives it.
- Call the office the morning of to schedule an in-office or telephone urgent care appointment.
- Update your OHIP card and notify the office when you have your new card.
- Patients who screen positive for COVID-like symptoms will be required to wear a mask at all times in the clinic.
