• Please call our patient services at 360-452-6252 to schedule an appointment for same day or future testing of potentially contagious illnesses. All testing is by appointment.
• Potential new patients need to provide a copy of all insurance cards, Social Security card, current driver’s license or valid identification card (parents for child), copy of tribal ID or CIB, and birth certificate for minors or dependent care prior to your first appointment.
• To schedule a dental appointment please call 360-452-6252 option #2.
• For dental emergencies, call first thing in the morning to check availability.
• If you need transportation for your appointment, please make arrangements at the time of scheduling or call 360-452-6252.
• When applying for medical travel assistance, please fill out a request and have it submitted 10 days prior to appointment for per diem. Failure to turn in medical travel form and proof of appointment 10 days prior to appointment can result in denial of per diem.
• To qualify for Medical Travel the following are required: A copy of your valid driver’s license, up-to-date vehicle registration, current vehicle insurance, verification of appointment date and time, up-to-date PAO-21, copy of Tribal ID or CIB, and must be PRC/CHS eligible.
• Travel forms must be submitted 10 business days before travel.
• Upon return of travel all receipts must be turned in or you will be denied for future medical travel assistance.
• If you lose a receipt, a lost receipt form must be submitted.
• If you have Medicaid insurance you will need to submit your driver license, car insurance and car registration to Fueldocs@paratransit.net and call 1-800-756-5438 to set up account and get fuel card and ferry card.
• Tribal Members are required to update a PAO-21 yearly to stay PRC eligible. Along with updated paperwork, you need to provide a valid Tribal ID and active health insurance.
• Verification of eligibility of payment for services is required prior to appointment. Failure to do so could result in services not being paid for.
• If you are experiencing a non-life-threatening medical emergency, you must access the after-hour nursing line at 541-529-9256 and get triaged before accessing emergency room services. If the nursing lines recommends the emergency room, it is your responsibility to call and notify CHS Coordinator at 360-452-6252 Ext 7613 within 72 hours of accessing emergency room in order for claims to be paid, failure to notify will result in denied claims.
• Walk-in Clinic may be accessed with approval of CHS Coordinator, to ensure no same day appointment are available at your Indian Health Facility, Lower Elwha Health Clinic. Failure to get approval will result in denied claims.
• All CHS eligible tribal members should have a True Scripts card to be used as a Secondary payer for prescriptions. If it is a new prescription, your insurance and/or True Scripts may require a prior authorization. This can take up to three days. If you have complications with your True Scripts card, call the customer service number on the back of the card. If you do not have a True Scripts card, call the PRC/CHS Coordinator at 360-452-6252 ext. 7613 and request a card.
