  • Potential new patients need to provide a copy of all insurance cards, Social Security card, current driver’s license or valid identification card (parents for child), copy of tribal ID or CIB, and birth certificate for minors or dependent care prior to first appointment.
  • To schedule a dental appointment, call 360-452-6252 option #2.
  • For dental emergencies, call first thing in the morning to check availability.
  • If transportation is needed for an appointment, make arrangements at the time of scheduling or call 360-452-6252.
  • When applying for medical travel assistance, fill out a request and submit it 10 days prior to appointment for per diem.
  • For medical travel, provide a copy of valid driver’s license, up-to-date vehicle registration, current vehicle insurance, verification of appointment date and time, up-to-date PAO-21, and copy of Tribal ID or CIB.
  • If you have Medicaid insurance for medical travel, submit your driver license, car insurance and car registration to Fueldocs@paratransit.net and call 1-800-756-5438 to set up account and get fuel card and ferry card.
  • Upon return of medical travel, all receipts must be turned in or future assistance will be denied.
  • If experiencing a non-life-threatening medical emergency, access the after-hour nursing line at 541-529-9256 and get triaged before accessing emergency room services.
  • If the nursing line recommends the emergency room, notify CHS Coordinator at 360-452-6252 Ext 7613 within 72 hours of accessing emergency room.
  • Walk-in clinic may be accessed with approval of CHS Coordinator.
  • For prescriptions, if you do not have a True Scripts card, call the PRC/CHS Coordinator at 360-452-6252 ext. 7613 and request a card.
