  1. 1Individuals must be receiving Fraser Health (FH) Palliative Care Services through a referral from a physician or health care professional.
  2. 2The Palliative Care Team will conduct an assessment and determine eligibility for palliative care services including admission into Hospice.
  3. 3Bills are sent out at the end of a patient’s stay or the end of each month to the person identified as responsible for financial affairs.
  4. 4Per diem payments may be made by cash, cheque or credit card, through the mail to the address provided on bill, or over the phone (using Visa or Mastercard).
