Patient Instructions:
  • New students are encouraged to submit immunization documentation prior to New Coug Orientation or the beginning of their first semester.
  • Graduate students are encouraged to submit immunization documentation prior to arriving on campus.
  • Ensure the file size of uploaded immunization records is not too large.
  • Ensure no special characters are in the file name of uploaded immunization records.
  • Add each dose of vaccinations, not just the most recent dose.
  • Ensure name and birthday are on every page of records.
  • Do NOT consolidate records; upload all records even if from different locations.
  • Handwritten records may be denied without a clinic stamp or medical signature.
  • When uploading proof of T-spot and QuantiFERON Gold tuberculosis testing, ensure date, location, and result are on records.
  • When uploading proof of skin test, include date and time the test was placed and read, clinic's location, and test result.
  • Keep an eye on the patient portal for communication regarding additional information needed.
  • Complete tuberculosis screening questions on the patient portal.
  • TB testing must be completed in the last 6 months and be a US-based test.
  • If receiving a vaccination at CHS, inform healthcare provider if you prefer information not to be uploaded to WAIIS.
  • Bring CougarCard or other identification for appointments.
Physical Directions:

Cougar Health Services is located in the Washington Building on NE Washington St, across the street from the Spark building. The Health Promotion door on the southeast side of the building, facing the residence halls and Stadium Way, should be used for workshops or Violence Prevention Programs.
