• Check your spam folder for email reminders.
• New clients should arrive 15 minutes early to fill out patient forms.
• If you are a new client for Massage Therapy, you will receive an email with a link to forms (Massage health history, Massage Consent, Direct Billing forms).
• If you are a first-time client for Chiropractic, you will need to fill out a second set of forms. Arrive early or print and fill them out before coming to the clinic.
• For WCB or SGI claims, see the SGI and WCB page for specific instructions.
• Please come prepared to pay for your appointment as claims may be audited.
• When booking with Janna, do not wear any perfume, cologne, or heavy scented body products due to severe allergies.
• For SGI Treatment, your Practitioner or Therapist requires your Claim Number, Date of Birth, Adjuster's Name & Phone Number prior to your first treatment. This information can be entered in the "Note" section of the Appointment Book or by contacting the clinic.
• For WCB Treatment, your Practitioner or Therapist requires your Claim Number, area of injury, employer, Adjuster's Name & Phone Number prior to your first treatment. Call the clinic to set up first-time WCB treatments; no same-day bookings for WCB Treatments.
