  • Bring picture ID (current driver license, state ID, or passport) for I-693 exam.
  • Bring Visa, Mastercard, or cash payment for I-693 exam (no checks accepted).
  • Bring immunization records if available for I-693 exam.
  • For I-693 exam, do not expect a second sealed envelope of original I-693 forms unless requested by USCIS; extra copies can be provided.
  • For My Chart access, acknowledge and accept terms before logging in; service is for non-urgent communications only.
  • Protect your User ID and Password for My Chart access.
  • Do not make inquiries for other patients without proper authority.
  • Do not make unauthorized inquiry, access, transmission, printing, or releasing of any patient information.
  • Do not install or download unauthorized software from the My Chart site.
