• Ensure X-ray and blood test appointments are scheduled after your medical examination with the panel physician.
• Bring your phone number, email address, and NB medicare (if available).
• Provide a list of all current medications and indicate if you use glasses or contacts.
• Bring your original travel passport and/or a government-issued identity document for identification.
• If available, provide your Medicare card (not mandatory).
• If applicable, provide your IME number or Client ID number, unless this is an upfront medical.
• Bring proper identification, eyeglasses or contact lenses if used, medical reports or test results displaying medical history, a list of current medications, and the medical report form (IMM 1017E).
• Clients over the age of 5 years will be requested to provide a urine sample during their assessment in the office.
