• Patients and visitors are required to wear a mask while in the clinic.
• For prescription refills, contact your pharmacy first; they will contact SMA for authorization if needed.
• For mail-order refills, ensure your information is current in the office.
• For urgent prescriptions, notify the clinic for immediate processing.
• Plan ahead for prescription refills for holidays, vacations, weekends, and travel.
• When requesting medication refills directly with SMA, include your preferred pharmacy.
• Use MyChart for non-urgent direct refill requests.
• MyChart should not be used for emergent medical conditions and symptoms; call the main number.
• MyChart is not monitored after hours, holidays/weekends.
• Always start a new MyChart message for each new issue or question.
• Do not send multiple MyChart messages to multiple staff members.
• Automatic payments will take place on the 1st of each month.
• Monthly fees are due on the first of the month for the previous month's services.
• Quarterly fees are due the first day of the quarter start.
• Semi-annual fees are due January 1st and June 1st.
• Annual fees are due January 1st.
• SMA membership does not provide health insurance coverage.
• Additional costs may be incurred for laboratory, medical imaging, surgery, specialist care, emergency department visits, and hospitalization outside of SMA's services.
• Patients are encouraged to combine SMA membership with appropriate health insurance coverage.
• Enrollment in an HMO Insurance Plan will severely limit SMA's ability to order and direct care.
• The beneficiary agrees to give up payment for all items and services furnished by SMA and to pay SMA the yearly enrollment fee without regard to any limits that would otherwise apply to what SMA could charge if receiving payment from insurance.
• Payment for items or services provided under the Seattle Medical Associates enrollment contract is the patient's responsibility.
• Claims for payment under patient's insurance for services at SMA should not be submitted.
• Payment limits under insurance do not apply to items or services provided under the Seattle Medical Associates enrollment contract.
• Other supplemental insurance plans may not pay for items or services provided under the Seattle Medical Associates enrollment contract.
• As an insurance beneficiary, you have the right to receive medical services provided by other physicians for whom payment would be made by insurance.
