• Ensure file is kept up to date with best phone numbers, home addresses, and emergency contact information.
• Inform receptionist of any changes to personal information upon arrival or by phone.
• Let receptionist know the reason for visit to help prepare and provide adequate time.
• For complete physical, Pap test, IUD insertion, or lesion biopsy/removal, tell receptionist as these require longer booking.
• Give 24 hours cancellation notice for appointments to avoid charges.
• All patients are expected to come back to follow-up on test results.
• If results are normal but condition persists, make an appointment for follow-up.
• If you would like to discuss results, book an appointment and inquire with office assistants to ensure results are on file.
• Keep personal information updated so the office can contact you.
• Be respectful of staff; zero tolerance for abuse.
• For problems associated with visit, contact manager in writing.
• For referrals, see family doctor for pre-consultation information gathering and investigations.
• For controlled medication, receive prescriptions from one doctor and fill at one pharmacy.
• Do not wait until the last minute to refill prescriptions at WIC or ER.
• If you may run out of medication, check with pharmacist if they can 'Adapt' your prescription.
• For prescription renewals, let doctor know during office visit; a dedicated follow-up visit may be scheduled for detailed medication review.
• If you need medical attention after-hours, visit Sea to Sky Walk-in Clinic or call 911 for emergencies.
