• Inform reception of any changes to address, phone number(s), and pharmacy.
• Take an active role in your own care by taking sensible precautions to keep healthy, using advice from care providers, asking questions, and telling us what is important to you.
• Be open and honest about your health history and needs (including past/current illnesses, allergies, medications, dosages, and family health history).
• Arrive at appointments promptly or notify us as soon as possible (at least 24 hours in advance) if canceling or postponing.
• Treat staff, other patients, and visitors with consideration and respect.
• Understand that violence, abuse (verbal or physical), or aggressive behavior will not be tolerated.
• Work with clinicians to build a relationship based on trust, honesty, respect, and a mutual desire to improve health outcomes.
• Refrain from discriminatory comments or actions.
• Be prepared to be seen by other STAR FHT staff if your usual provider is not available or be willing to wait.
• Understand that staff may prioritize time for emergency or high-need situations.
• Understand that STAR FHT has limitations in services provided.
• Understand that posting harassing, bullying, or defamatory comments on social media is unacceptable and could result in immediate dismissal.
• Bring a valid health card to every visit; you will be charged if informed once about an expired health card.
• Dress appropriately for the area being examined (e.g., wear a top that exposes the shoulder for a sore shoulder examination, or change into a gown).
