• Bring ALL medications (original bottles or Blister Packs) to each visit.
• Contact your pharmacy to initiate prescription renewals.
• Contact your pharmacy at least 2-3 weeks prior to running out of medications.
• For appointments, 1 or 2 medical issues can be addressed per visit; additional concerns may require new appointments.
• If arriving late, you may have reduced appointment time or need to reschedule.
• Give as much notice as possible when canceling appointments.
• Complete all patient sections of forms before submitting them for completion.
• If you miss your appointment, you will be charged a $10 fee.
