Patient Instructions:

Keep family's information up to date in DEERS.
Provide a copy of off-post vaccine records to your provider and care team.
Contact primary care clinic for COVID-19 testing information.
Have DOD ID number when presenting for a COVID-19 test.
Check MHS GENESIS Patient Portal for test results.
Notify chain of command and self-isolate while awaiting test results.
Isolate for five-day period if test is positive, in accordance with Force Health Protection Guidance Supplement 15 (revision 3).
Stay in a separate room from other household members, if possible.
Don’t share personal household items and wear a well-fitting mask when around others.
Ventilate the space as much as possible.
Utilize a well-fitting mask for an additional 5 days after leaving isolation when around others.
