  • Wear or bring comfortable clothing to change into for initial assessment and subsequent treatment (shorts for ankle or knee assessments – a tank top for neck or shoulder assessments).
  • Come approximately 10 minutes before your first visit.
  • Arrive 10-15 minutes early to fill out intake paperwork.
  • Print and fill out the intake form before arriving.
