If paying cash, full payment within 90 days from first statement date entitles to a 35% discount.
If paying cash, full payment within 365 days with regular payments entitles to a 25% discount.
Payment terms from 13 months to 18 months with no missed payments will be eligible for a 10% discount.
Be aware of the need for prior authorizations required by your insurance policy.
Contact Patient Services Representative for information on programs if unable to pay in full within 90 days.
