• Please come to the clinic if you wish to discuss your results or if your symptoms persist or worsen.
• You may book a phone consultation to discuss your results.
• If you have any preference like a specific doctor or location for specialist referrals, please let the receptionist know.
• Call the clinic if you do not receive any update after 2 weeks regarding a specialist appointment.
• If you want your information disclosed to a family member or someone, you need to provide written permission or bring them with you to the clinic.
• If you request your records sent to your family doctor, written consent is needed and charges will apply.
• If you do not have OHIP, please indicate during registration if you have private insurance or self-pay.
• For services not covered by OHIP or private insurance (e.g., medical certificate, sick note, form filled out), talk to the receptionist for more information.
• If you are unable to reach the receptionist by phone, leave a voicemail with your name, phone number, and reason for the call.
• You can also email info@danforthmedicalclinic.ca for a response within 1-2 business days.
• For very urgent or emergency situations, call 911 or proceed to the nearest hospital emergency department.
• Call the clinic to ask about doctors' schedules, but be aware schedules may change without prior notice.
• Call the clinic to ask details regarding TB testing.
• Call the clinic to ask for the availability of vaccines for children.
