• For Sports Medicine, a referral from a family doctor or walk-in physician is required.
• Arrive at least 15 minutes prior to your appointment to fill out paperwork and/or to change into appropriate attire (shorts for back/lower limb, tank tops for neck/upper limb).
• A minimum of 24 hours notice is required for all cancellations without penalty.
• No-show fees will apply for all booked appointments.
• If arriving late to a booked appointment, the full appointment time may not be available and re-booking may be required.
• The clinic does not treat patients with workplace or motor vehicle injuries.
• For Civil Aviation Exams, arrive at least 15 minutes prior to your appointment to fill out paperwork.
• A minimum of 24 hours notice is required for all cancellations without penalty for Civil Aviation Exams.
