Patient Instructions:
  • Bring report cards from the last 12 months.
  • Bring any reports prepared by professionals who have seen your child, including previous educational assessments, reports from Occupational Therapists, Speech and Language Pathologists, or other medical professionals.
  • If your child takes ADHD medication, they should take whatever medication they take for a regular school day on testing days.
  • Cancellations must be made 24 hours prior to your appointment date.
  • Questionnaires must be completed on the first day of testing. If a parent is not able to bring their child in, the questionnaires can be sent in advance but must be brought in on the first day of testing.
  • If a parent is not bringing the child in, they must send a letter outlining the child's date of birth and grade.

Located on West 41st Avenue. Pay parking is available underneath the building, with entry from 41st Avenue. One-hour street parking is also available in the vicinity.
