  4. 4Drink 2 glasses of water 1 hour before IUD Treatment, do not empty your bladder.
  5. 5Upload Canadian Migraine tracking diary to AVA Connect at least 2 days before Botox migraine treatment.
  6. 6Bring your list of medications, health history, and comfortable clothing for Surgery Consult.
  7. 7Confirm your appointment via text, email, or AVA CONNECT account.
  8. 8For urgent prescription refills, select a "Regular in person" appointment.
  9. 9Patients seen on a SameDay appointment must book a follow up appointment 1-2 weeks after being seen.
  10. 10For Mental Health appointments, complete forms sent via email 2 days prior.
  11. 11For Insurance forms, complete the Medical "Insurance History form" and bring your insurance forms to the visit.
  12. 12For Migraine Consult, complete 4 forms (Migraine History, Midas, Hit 6, Canadian Migraine Tracker) 1 week before appointment.
  13. 13For Migraine FU In person, complete Migraine Follow up form and upload Canadian Migraine tracker to AVA Connect 2 days prior.
  14. 14For Migraine FU Virtual, complete Migraine Follow up form and upload Canadian Migraine tracker to AVA Connect 2 days prior.
  15. 15If feeling unwell, bring and wear a face mask if possible.
