Referral Process: Niagara Children’s Centre accepts referrals from parents/legal guardians, physicians, community service providers, and school boards (with parental consent).
How to Refer: Online Service Request Form is preferred for most referrals. Physicians can also fax a referral form. EarlyON staff and Designated Child Care Centres have specific referral portals.

  • Physician/Primary Care/Healthcare Providers Referral Form (if faxing). The Niagara Children's Centre Birth to School Start (OT| PT | SLP) Referral Checklist must accompany the referral form if requesting OT
