Instructions

  1. New patients can sign up for Psychiatry and Counseling appointments by stopping by or calling health centers that offer these services.
  2. For Medical Primary Care and Dental appointments, new patients should sign up for the online Integrated Case Management intake process.
  3. Established patients can call (303) 293-2220, arrive at health center opening, or schedule appointments online through the patient portal for same-day appointments.

Getting Here

  1. 48th Avenue Health Center is located inside the Denver Rescue Mission.
  2. The main office on Champa Street has two-hour metered parking in front and free one-hour parking on 21st Street between Broadway and Curtis Street.
