Referral Information for CASA Child, Adolescent and Family Mental Health

Referral Process: Caregivers of children aged 2 years 9 months to 4 years 9 months can self-refer. Physicians referring children in this age group should have the family fill out the intake form. For children and youth aged 4 years 10 months to 17 years, referral is through Recovery Alberta. Many programs require a referral from a physician, therapist, or the child's school. The Trauma program in Calgary requires referral by a mental health professional.
How to Refer: For 3-5 year olds: Fill out and send/drop off/fax the referral form to CASA Centre or Fort Road. For 5-17 year olds: Call Recovery Alberta intake services for a phone interview. For Calgary Trauma Program: Speak to a mental health professional and direct them to the referral form.
