  1. New patients should provide contact information, date of birth, gender, Alberta Health Card, list of current prescription medicines, medical history, family physician, date of laser corrective eye surgery (if applicable), ophthalmologist (if applicable), health benefits/health insurance policy and group information, current prescription eyeglasses (if applicable), current contact lens brand/type (if applicable), and location of most recent eye exam.
  2. A minimum 24 hours notice is required for appointment cancellation, otherwise a $75 fee will be applied.
