Instructions

  1. Bring an up-to-date list of medications (name, dose, doses/day) and supplements; do not bring medication bottles.
  2. If you have a blood pressure cuff, record 3 readings and average the last two; write down heart rate and blood pressure.
  3. Turn off radio or TV during telephone encounters.
