Patient Instructions
  1. 1For in-person visits, sanitize hands and maintain 6 ft/2m distance.
  2. 2Once registered, you may be directed to a secondary waiting room or advised to wait in your vehicle/outside.
  3. 3The physician will assess you by phone and determine if an in-person visit is needed.
  4. 4For virtual care, ensure you have a valid OHIP card (with current version code) or be prepared for a paid visit.
  5. 5For virtual care, you must have been seen by the physician or at the daytime clinic within the past 2 years (exceptions apply).
  6. 6Log onto the virtual care site (powered by Insig) prior to your scheduled appointment time.
  7. 7For patient portal registration, confirm your email address (check spam/junk/clutter folders).
  8. 8To view test results and modify appointments via patient portal, sign a Patient Consent Form in person or email it to info@onyxrx.com.
  9. 9If concerned about normal/negative test results, come in during operating hours to discuss with a physician.
  10. 10Children must be supervised by a parent or guardian at all times.
  11. 11Services not covered by OHIP must be paid for at the time of service.
  12. 12Patients with invalid or expired health cards are required to pay a $50 Hold-Payment charge, which is reimbursed upon valid health card presentation.
  13. 13Disruptive, rude, or aggressive behaviour will not be tolerated.
