Patient Instructions
  1. 1No additional individuals (family, children, etc.) are allowed to accompany the person seeing a doctor, undergoing a test and/or procedure.
  2. 2No walk-ins are accepted.
  3. 3All patients are required to self-screen for exposure and/or symptoms of COVID-19. If exposed or symptomatic, quarantine and call the clinic ASAP.
  4. 4Masks are required inside the clinic (masks are provided if you do not have one).
  5. 5Bring your valid OHIP card (Ontario Health Card) – if cards are not up-to-date, please reschedule your appointment, or pay for the office visit.
