  2. 2Each appointment has a time slot dedicated to a single issue. Please schedule another appointment if you need to discuss any additional issues or concerns.
  3. 3Please call the office to book an appointment to discuss the results of any tests on investigations you have had.
  4. 4For medication refills, please note we typically refill prescriptions in 3 monthly cycles except when otherwise expedient. Please book an appointment to review/refill your medications 1 week before you run out of them.
  5. 5Kindly ask your pharmacy to fax a refill request to us ahead of your appointment.
  6. 6Applicable fees will be charged for services not covered by OHIP. This can include the completion of certain forms, ear flushing and doctor's notes/letters.
  7. 7Please notify us of any appointment cancellation at least 24 hours prior to avoid a $50 cancellation penalty that may be applied.
  8. 8Please be aware that you may not be seen at your exact appointment time, as wait-times to be seen may invariably be impacted by unpredictable complex care needs that would require longer time than intended to resolve.
