Patient Instructions
  1. 1Appointments must be made over the phone.
  2. 2Show up on time for your intake appointment, and all appointments to follow.
  3. 3Failure to collect medication from the pharmacy for 2 successive days requires medication to be withheld until reviewed by prescribing physician.
  4. 4No replacement of any lost, stolen, or spilled medication.
  5. 5Medication may not be picked up early from the pharmacy.
  6. 6Urine samples will be collected on a random basis; failure to provide a sample when requested will be considered the same as a positive urine sample for opiates.
  7. 7Patients who are pregnant, or are planning a pregnancy, must inform the Methadone or Suboxone prescriber immediately.
