  1. 1A valid Ontario health card must be presented for each person to be seen by a physician or nurse.
  2. 2Renew OHIP coverage 4 to 6 weeks prior to expiry.
  3. 3Book next follow-up appointment before needing medication refills.
  4. 4Call office for follow-up appointment a minimum of 2 weeks prior to needing medication refills.
  5. 5Ask for medication repeats when seeing doctor or nurse practitioner for another reason.
  6. 6If running out of medication, ask pharmacist to fax office with refill request (allow 3-4 business days).
  7. 7For urgent medication needs (non-narcotics/controlled substances), visit the walk-in clinic on the Ground level.
  8. 8Ask pharmacist to extend chronic medication (at their discretion).
  9. 9Appointments cancelled without 24 hours' notice are subject to a $50 charge.
  10. 10If doctor/nurse practitioner asked you to book an appointment to follow-up on results, book prior to leaving the office.
  11. 11If you have had an investigation and are concerned with the result but haven't heard from the office, call to follow-up on status.
  12. 12If referred to a specialist for a non-urgent reason and haven't heard back within 30 days, call the office.
  13. 13Bring test requisitions to your test(s).
