Instructions

  1. SLMC will charge for services that are not covered by OHIP at rates suggested by the OMA.
  2. There is a charged of $50 for a missed regular appointment (in person or virtual, or if you cancel without the required 24 hours) and $100 for missed annual review appointment.
  3. Face masks are NOT mandatory but strongly encouraged when in the clinic.

Parking Information

On-site parking spaces are limited. Parking fee is $5 (or $3 for some services). A parking token can be purchased at the front desk. Other large parking lots are available in the area within walking distance. Free parking for laser clinic clients.
