Patient Instructions
  1. 1SLMC will charge for services that are not covered by OHIP at rates suggested by the OMA.
  2. 2There is a charged of $50 for a missed regular appointment (in person or virtual, or if you cancel without the required 24 hours) and $100 for missed annual review appointment.
  3. 3Face masks are NOT mandatory but strongly encouraged when in the clinic.
  4. 4If you think you have Coronavirus (COVID-19) symptoms or have been in close contact with someone who has it, please call before coming to the clinic.
  5. 5For Life Lab services, no appointments needed.
  6. 6For family practice, appointment only.
  7. 7Please notify reception staff when you arrive if you are here to see the nurse for an injection such as a flu shot, allergy shot, TB test or other vaccination.
  8. 8Completing and submitting the new patient intake form does not automatically make you a patient at this clinic. Only accepted patients will be contacted.
  9. 9Please do not transfer your medical records until after you have been accepted as a patient.
  10. 10Incomplete new patient forms will not be considered.
  11. 11For registered patients, complete the online form and staff will contact you to arrange an appointment in the next 2 BUSINESS DAYS.
  12. 12In emergency cases, please call the clinic, present to the Walk-In clinic, or present to the nearest Emergency Department.
  13. 13Patients not currently registered with a Family Physician at St Laurent Medical Centre will not be contacted for appointment requests.
